Better diabetes care closer to home

There is always opportunity to improve services for patients. Many of our diabetic patients are very knowlegeable about their condition. Better understanding of the condition leads to better self-care in diabetes. In many parts of the country there are good intermediate care services available (better working with our specialists in the community). We would like to move our locality diabetes care in this direction.
